Breast Implants: Smooth vs Textured Surface

Breast implants are made with either a smooth or a textured (rough) surface. The argument for textured implants rests on the supposition that tissue in-growth from the breast will allow the implant to incorporate itself better and therefore lead to less capsular hardening and contracture. Unfortunately, this theory has yet to be conclusively proven in practice. Some research papers in the early 1990's showed that textured silicone implants hardened less often than smooth ones. However, it does not appear to be the case for saline implants. While some studies show a small difference, others studies show no significant difference at all. What many surgeons have notices is that textured implants tend to "ripple" more. In most cases, smooth is better. However, in a patient that prevents for implant exchange due to capsular contracture with a smooth implant, I tend to switch over to a textured implant (or vice-versa) to change one variable.
