Breast Implants: Behind or In Front of the Muscle

The breast implant can either be placed in-front of or behind the pectoralis major chest muscle. In 95% of primary (first time) breast augmentation I place the implant behind the muscle. There are several advantages. First of all, there seems to be a lower rate of hardening. Secondly, there is better preservation of sensation to the nipple. Third of all, the edges of the implant blend more naturally behind the muscle leaving a more natural contour. Lastly, there is improved visualization on mammogram. There are, however, some disadvantages. Most importantly, there is significantly more pain after the surgery. Secondly, the breasts appear fuller at the top during the first few months until they drop into position. Lastly, in active athletic women (body builders especially) the implants can occasionally "bounce" when the chest muscle is contracted forcefully (as in some body builders). Despite these inconveniences, behind the muscle is usually the better choice.
